Short-Term Rehabilitation Overview

Bridging the gap between hospital and home.

Whether as a result of trauma, disease, stroke, surgery or chronic condition, when mobility, functionality or cognitive abilities are impaired, a period of rehabilitation is often the pathway to recovery, renewed independence and the best possible quality of life.

At OPIS, our in-house SigmaLink Rehab department is among the area’s finest.   Available to long-term and short-term customers alike, our comprehensive rehab services include a wide range of physical, occupational and speech therapies, as well as innovative treatment protocols. Keeping with our personalized approach to care, rehab programs are tailored to each individual customer’s specific capabilities, limitations, needs and goals.

The SigmaLink Rehab staff is composed of highly-trained, dedicated rehab professionals, all with extensive experience, including the specialized care required by many elderly customers. Rooted in the OPIS tradition of excellence in long-term nursing care, this unique level of experience and expertise enables us to better respond to the often complex challenges of geriatric rehab and enhances our ability to restore individuals to the highest levels of form, function and independence…and in a majority of cases, return home to their friends and families.

Cardiac Rehabilitation

For those recovering from a heart attack or heart-related disease, heart surgery, and/or coronary intervention procedures such as stenting and angioplasty, our comprehensive cardiac rehabilitation services can help customers —

  • Achieve optimum recovery
  • Increase cardiovascular fitness
  • Reduce cardiac symptoms
  • Improve overall health and
  • Reduce the risk of future heart problems

Provided through our in-house Sigma Link rehab department, individually tailored programs are designed, supervised and implemented by an experienced interdisciplinary team of specialists that may include:

  • Physicians
  • Nurses
  • Therapists
  • Nutritional consultants
  • Mental health professionals…

all working together to help customers return home, get back to families, friends and jobs, resume normal activities and effectively manage their heart conditions and issues.

Discharge Planning

Prior to leaving the center, our team of rehabilitative professionals makes certain that each customer is ready for the challenges involved with returning home or to an alternate level of care.

In addition to creating personalized home exercise plans and guidelines for safely performing daily routines, we’ll make a complete assessment of the home environment and take whatever steps may be necessary to ensure that he or she can manage stairs and other potential problem areas. Social workers may arrange for wheelchair assistance or other assistive devices, while nutritionists will review dietary issues that may impact recovery. If applicable, our nurses will also provide information and training on wound care and medication management.

The overriding goal of this comprehensive program is to pave the way for customers to make a seamless transition back to their homes and families, fully prepared to resume daily life with the highest possible level of function, safety and independence.

Stroke Rehabilitation

The goal of our stroke rehabilitation program is to help customers relearn lost skills, regain independence and improve quality of life.

The severity of stroke complications and each person’s ability to recover lost skills and function varies widely. As a result, each care plan is custom-tailored to achieve optimum results and may include some or all of the following activities, depending on the part of the body and/or ability affected:

  • Physical, occupational and speech/language therapy
  • Strength and mobility exercise therapy
  • Range-of-motion therapy
  • Gait and balance training
  • Psychological support
  • Dysphagia therapy
  • Functional electrical stimulation
  • Psychological evaluation and treatment

Importantly, we continue to adjust and adapt the stroke rehabilitation plan during recovery as the customer relearns skills and his or her needs change. With patience and perseverance, the great majority of those in our care continue to make gains over time.

IV Therapy

When the road to healing and maximum recovery calls for intravenous therapy, our nursing team ensures that this important service is done correctly, comfortably, and with compassion.

From maintaining the right fluid balance, to monitoring drug effectiveness, to treating infection and easing pain, highly-skilled IV therapists and nursing specialists work closely with our customer’s personal physicians to provide relief, reduce stress and speed recovery.

The many conditions that we may treat with IV therapy include:

  • Congestive heart failure
  • Gastrointestinal disorders
  • Infections

Neuro-Muscular Therapy

For those affected by Multiple Sclerosis, Parkinson’s Disease, Guillain-Barre Syndrome and other neuro-muscular disorders, SigmaLink Rehab’s highly specialized neuro-muscular therapy provides a pathway back to optimum performance, function and overall well-being.

Under our unique program, personalized care plans are designed to address each customer’s often co-occurring physical, cognitive and emotional needs, utilizing a wide range of coordinated services, techniques and treatments, including:

  • Strength and range of motion exercises
  • Mobility training
  • Re-education in skills of daily living
  • Speech and voice re-training
  • Swallowing management
  • Gait and balance training
  • Psychological counseling and family support

Orthopedic Rehabilitation

A recognized leader in the field, our Sigma Link orthopedic rehabilitation program offers the combined therapeutic, clinical and technical resources needed to address a wide range of orthopedic conditions, including:

  • Joint replacement
  • Fractures
  • Cervical and lumbar injuries
  • Arthritis and osteoporosis
  • Amputations

Using a collaborative, multidisciplinary approach to care, individually designed, supervised and implemented treatment plans combine specialized therapies, pain management, strength, balance and mobility training, biomechanical re-education, joint protection and other techniques to help restore even frail and high risk patients to the highest possible levels of functional independence.

Peritoneal Dialysis Care Program

For short-term rehab customers with kidney failure or other conditions that require renal support, our Peritoneal Dialysis program offers the highest level of care and flexibility.

With peritoneal dialysis, you have some important choices to make in treating your condition. Primarily, these involve whether to receive your dialysis at our center or give yourself treatments at home. However, peritoneal dialysis outside the center isn’t an option for everyone with kidney failure. Obviously, you will need full manual dexterity and the ability to care entirely for yourself or with the help of a reliable caregiver.

If you decide to treat yourself at home, it is especially important that you work closely with your health care team, which includes you and your family. Only by becoming fully informed about your treatment and working as a team can you achieve the best possible results and lead a full and active life.

For many, dialysis care at our center is the preferred option for a variety of reasons. Featuring expert clinical supervision by a Board Certified Nephrologist, the program includes:

  • Skilled dialysis unit nurses familiar with each individual’s condition and needs
  • Direct nursing interaction with nephrologist
  • Specially designed treatment setting
  • Individual treatment planning by a multi-disciplinary team
  • Specialized diets formulated by a registered dietician
  • Customer personal equipment use promotes convenience and confidence
  • Highly-trained, full-time staff
  • Comprehensive social service support

Physical, Occupational & Speech Therapies

Available to long-term and short-term customers alike, our comprehensive physical, occupational and speech therapies and innovative treatment protocols address a wide range of orthopedic, neurological and other disabilities, diseases and conditions, including:

  • Joint replacement
  • Neuro-muscular diseases
  • Stroke
  • Dysphagia
  • Fractures
  • Post-surgical complications
  • Diabetes
  • Amputation

In keeping with our personalized approach to care and caring, therapy programs are tailored to each individual customer’s specific capabilities, limitations, needs and goals. Unlike many facilities, therapies may also be provided as often as the customer’s physician recommends it – up to seven days a week – helping to speed healing and recovery.

Importantly, our Sigma Care staff is composed of highly-trained, dedicated rehab professionals, all with long experience, including the specialized care required by many elderly customers. Rooted in the OPIS tradition of excellence in long-term nursing care, this unique level of experience and expertise enables us to better respond to the often complex challenges of geriatric rehab and enhances our ability to restore individuals to the highest levels of form, function and independence… and in a majority of cases, return home to their friends and families.

Post-Surgical Rehabilitation

Following a surgical procedure, our overriding rehabilitation goal is simple: to restore post-operative customers to their highest level of strength, mobility and function as quickly and safely as possible.

It’s a goal our multidisciplinary rehab team of therapists and support specialists consistently achieve through personalized care planning and services targeted to each individual’s unique needs, abilities and limitations.

Incorporating innovative therapies, treatments, activities and support, post-surgery rehab programs address a broad range of conditions and diagnoses, including:

  • Joint replacement
  • ACL replacement
  • Rotator cuff repair
  • Lumbar/cervical laminectomy/discectomy
  • Spinal fusion
  • Hand surgery

Respiratory Rehabilitation

For people with lung disease, cardiopulmonary disorders and other chronic respiratory problems, including–

  • Asthma
  • Chronic bronchitis
  • Compromised heart function
  • Emphysema
  • Obstructive sleep apnea

our comprehensive respiratory rehabilitation program is often a key to more complete recovery and the ability to better cope with their conditions.

Physical therapists, occupational therapists and speech pathologists utilize a wide range of innovative therapies, often supplemented by training in breathing control techniques designed to promote relaxation and alleviate fear and tension.

Each customer’s individually-tailored program may also include oxygen therapy, bronchodilator therapy, inhaled medication delivery, dietary counseling, as well as personalized instruction on how to manage his or her respiratory condition and effectively deal with self-care requirements where it’s most comfortable and convenient – at home.

Transitional Care

Our highly flexible Transitional Care program is a temporary private-pay option for customers who may benefit from an additional period of professional care following a prescribed course of rehabilitation.

This unique offering combines skilled nursing, round-the-clock professional supervision and dedicated personal assistance, all in a familiar and restful environment.

Adaptable and responsive to individual goals and needs, each customer decides when the time is right to return home. Not only is transitional care a perfect way to rest, recharge and restore confidence, for many it provides an opportunity to sample full-time residency without making a long-term commitment.

Wound Care

An all-too-common health care problem, hard-to heal wounds can present major obstacles to optimum recovery. Based on that recognition, our exceptional wound care program is designed to treat those experiencing a wide range of problems, including:

  • Diabetic ulcers
  • Pressure ulcers
  • Arterial and venous ulcers
  • Burns
  • Infected wounds
  • Non-healing surgical wounds
  • Amputations
  • Complex soft tissue wounds
  • Traumatic wounds

Specialized services also address complex wounds resulting from conditions that compromise circulation, such as diabetes, traumatic injury, surgical breakdown, congestive heart failure, vascular disease, incontinence, obesity, renal failure and lymphedema.

Our highly-experienced, full-time staff includes a wound care physician-specialist and team of nurses, dieticians and therapists, who assess, diagnose, oversee and administer all wound care procedures and protocols. Among the many leading-edge therapies and treatments we offer are:

  • Surgical wound debridement
  • Chemical wound debridement
  • Burn care
  • Diathermy
  • Tissue ultrasound
  • Electrical stimulation
  • Whirlpool therapy

Our unique whole person approach to care also focuses on wound prevention education and equipping soon-to-be-discharged customers with the self-management skills and tools needed to avoid complications and reoccurrence.
